Below are steps on how to book an appointment:

• Click the View Appointment Form button below.
• Fill-in all required fields in the Appointment Form.
• Send your appointment details by clicking the Submit button.
• You will receive an Appointment Number in your email account. Make sure to type your email address correctly.
• We will email or call you to confirm your online booking/appointment.
• Present your appointment number to our triage staff on your scheduled visit.
